How to Enroll/Register your Child (Kindergarten, New to District) at Middletown City Schools

Kindergarten, New to District

(Kínder, Matrícula abierta, Nuevo en el Distrito)

On May 18, we will open up in-person registration and enrollment. Due to COVID-19, we will not have registration and enrollment at our Central Office. Instead we will have hours at our elementary schools and the Middletown High School (see rotating dates below).

HOURS OF OPERATION  (If you arrive after 3:30/5:30 PM, we will not admit you inside.)

  • August: 7:00 AM - 5:30 PM (times vary based on location, scroll down)

While we encourage you to enroll at your home school building, parents/guardians can register at any location. If you are registering a high school student, you can do so at the Middletown High School OR any of the below elementary locations. If a parent/guardian has multiple students in multiple buildings, they may register all of them at the same location. We will be open rain or shine (plan accordingly!).

Enrollment/Registration will take place at the following locations (in the cafeterias):

  • August 3: Amanda Elementary (9 AM - 5:30 PM) & Creekview Elementary (7 AM - 3:30 PM)

  • August 4: Mayfield Elementary (9 AM - 5:30 PM) & Miller Ridge Elementary (7 AM - 3:30 PM)

  • August 5: Rosa Parks Elementary (9 AM - 5:30 PM) & Wildwood Elementary (7 AM - 3:30 PM)

  • August 6: Middletown High School Community Room (7 AM - 5:30 PM)


 Stay Safe Ohio: What’s expected?

  • Required: face mask/covering;

  • We will only admit five (5) people in the cafeteria at a time;

  • Do not come in groups: one (1) person per family with custody (only a custodial parent or legal guardian can officially enroll a student);

  • Leave children at home;

  • If you or someone in your household is sick, stay home;

  • If you haven’t already, you will need to start the registration process online: https://bit.ly/2xyfBln 

What should you bring with you?
All documents must be originals (We will make copies at registration)

  • Student Birth Certificate

  • Student Immunization Record

  • Parent Photo Identification

  • Proof of Residency

  • Custody Documents, if applicable

Things to note:

  • Kindergarteners MUST have shot records;

  • If you are coming from another district and your student has an IEP or 504, bring a copy;

  • If you are registering a high school student, you can do so at the Middletown Middle School OR any of the below elementary locations;

  • If you apply for open enrollment, we will notify you by July 15, 2020. The delay is due to COVID-19;

  • Due to COVID-19, we will not have registration and enrollment at our Central Office. We hope to open up the Central Office in August 2020.
