Prom Guest Information

Prom Guest Applications can be picked up in the class office (Guest forms are for any non MCSD student)
Guests may not be a Freshman or any grade lower NOR more than 20 years old. Sophomores must be escorted by a MHS Junior or Senior.
Completed forms and a copy of guest ID is due by Thursday, April 18 at 3 p.m. NO LATE FORMS WILL BE ACCEPTED.
